Pituitary Carcinoma (Pituitary Neuroendocrine Tumor)

Pituitary carcinomas are rare, with only 12 cases occurring in a series of 7,602 pituitary lesions (48). The term carcinoma can only be applied to adenohypophyseal neoplasms after they have produced metastases, either systemically or through the cerebrospinal fluid. Most cases express hormones, typically prolactin or ACTH, and progress from macroadenomas. Prognosis for these lesions is poor, with 66% mortality within 1 year of diagnosis in one series (71). No known histologic parameters can predict which cases of pituitary adenoma will metastasize, so diagnosis of this entity depends on neuroimaging studies.
